For at least 2 hours we have not received any external emails. All emails in the reports appear to process as normal, and external emails show as "successfully delivered" but the emails are never received.

You on m365/o365? If so, you done a message trace to make sure message are not reaching the m365/o365 gateway?
thank you for the suggestion - i did run a trace and the emails do not show up in the o365 message trace.

Do you have any other device acting as an MTA after Sophos Email?
What mode is Sophos Email deployed?
