What is the macOS version and Sophos core agent version running on the affected device? Is the issue happening on just one device?
I also suggest starting with the component isolation. This would help us pinpoint if there's a single component that's causing the performance issue. You may refer to the following article for reference. This article is for Windows devices, but the steps are just similar for a Mac.
