PureMessage for Exchange 2013? Only scheduled scan?

From the announcement of PureMessage 4.0 for Exchange 2013 (http://blogs.sophos.com/2013/07/23/puremessage-for-microsoft-exchange-2013-pme-4-0-now-available/):

The retirement of the VSAPI means that PME 4.0 now provides background scanning (scanning the information store after every virus identity, or IDE, update) of Exchange private and public information stores using the Exchange Web Services API (EWS API). In following the Microsoft recommendation to scan mail in transit, PME 4.0 does not support the legacy proactive and on-access information store scanning modes.

Can someone explain to me the implications of the last phrase?

What happens if I receive an email with a virus, or spam? Will it only get scanned for  spam and virus scan during the next scheduled background scan?
