Attachments injected into e-mail after sending

I have an interesting security issue I'm looking into. A user on our network replied to a customer inquiry via e-mail. The customer responded back that the reply had pornographic attachments. Is it possible that the e-mail was hi-jacked in transit? I've never seen this happen before. Any insight would be most appreciated. Thanks, RobM
