The upgrade from Pure Message Exchange 4.0.2 to 4.0.4 is a direct upgrade which can be run over top of the existing installation. You can find further details and release note information on the following page.

I am not aware of any known issues with the upgrade, though you can find Sophos' full KIL list on the following page. A few are present but further steps are detailed within the document as well.
