The test email is sent but, when we add people to the campaign and try to send it the email shows it's sent but never hit's the clients inbox. It's been 4 days.

It would be worth checking to ensure that the domains and IP addresses used for Phish Threat campaigns are allowed in your environment.
Are the emails being shown as sent in Sophos Phish Threat but not appearing in user inboxes or not showing sent at all?
If Dianne's suggestion does not solve the issue I would advise raising a case up with support, enabling remote assistance and providing them with the name of your Campaign. From there they can trace if the emails have been sent out and provide you back with any SMTP server response codes.
