I have configured infected items to be quarantined but when I upload the Eicar test file, it never lands in quarantine. Why?

SharePoint settings are to attempt to clean files. Antivirus action is to quarantine and replace text. When I upload the eicar file, the file will show the replacement text but never be quarantined. This happens on SharePoint 2010 and 2013 farms. What am I missing?
