Hi homerjs
Route inbound mail through gateway: You may use this option or setting to route inbound mail through specific gateway IP addresses when the auto MTA firewall rule has a NAT policy.
Detail information: By default, MTA auto generated mail has default NAT policy MASQ applied which will use Interface default IP.
In some scenario or setup customer want to SNAT traffic for email over MTA rule if MX record pointed to Alias IP/specific IP rather than WAN Interface IP. So, they will choose or apply the custom NAT on auto MTA rule.
However by default that SNAT will be applied to outbound email only. If you want to apply the same SNAT IP address to SF MTA Inbound email or would like to reflect that SNAT IP inside email header, then you must need to use check “Route inbound mail through gateway”.
