Sophos UTM 9 occassional 504 gateways & 408s

We have a Sophos UTM 9 AWS EC2 protecting our AWS application servers. ur current configuration is based on Chad Ingles Youtube series starting with this video https://www.youtube.com/watch?v=TlHZBlRpXyU. Over the last several weeks we've had what seem like random outages that we can't figure out. Our application server gets slow and for the most part, unresponsive. While it's in that state the Sophos reverseproxy logs are littered with 504 and 408s. The application server logs during that time show evidence that outgoing traffic doesn't make it and instead times out. Rebooting the application server eliminates the problem temporarily but we see it return every 2-3 days. 

I'm not sure where to look at this point but it seems to be pointing at a networking issue. Could anyone offer any insight? Could this potentially be a Sophos problem or does it sound like an Application server issue? Let me know what logs or other information you may need.

    this sounds like an application server issue. What is the load on the app server, is there enough memory (should not swap), are there enough free web worker, ...
