Wireless -> Hotspot -> Hotspot Customization: how to use login page templates/Images/Stylesheet

I need to know how to use the Hotspot Customization, with login page template, images and stylesheet. 

I upload these files but neither logo (image) nor login page are shown correctly. 

Also, I have a big doubt between captive portal from Authentication and captive portal from Hotspot.
