If the login item in the Users and Groups settings is greyed out, it is likely because those settings are locked. Make sure that the lock icon in the lower left corner shows a lock in the unlocked position. If not, click the closed lock, then enter your password to allow changes, then the minus sign next to the Sophos UI login item should be usable (no longer be greyed out).
Yeah, I don't have guest enabled and I don't have a login item. I am running Lion 10.7.4 on a MBP-Retina. Still have two icons.
None of the recommended steps had any effect. The only thing that did work was that Applescript fix, but that is a ridiculous workaround. This has been going on for far too long to still be an essentially unadressed problem.

That worked for me until yesterday. Using Mountain Lion on a Mac Pro, the double icon issue seemed to be resolved a couple of weeks ago in that while two menu bar icons would appear at startup, after the first update, whether automatic or manual, one of the icons would disappear until the next reboot. Updating has always worked. But yesterday, Sophos went through its update process starting with two icons, and after the update process was finished (which included both icons showing the updating arrow and then disappearing), the two icons were still there. While it's more cosmetic than anything else, it doesn't inspire confidence.
At the same time, running Lion, the double icon issue stopped a while ago and has yet to return.
Opening up the 'Sophos Anti-Virus' application, then going to the 'Sophos Anti-Virus' menu and unchecking 'Show Status in Menu Bar' so they are removed, closing the app then repeating the process so that the icon is placed back in the menu worked for me.
