I've found a bug with the Request Redirection in combination with Reverse Authentication.
Without Reverse Authentication the redirection is working as expected. If I turned on Reverse Authentication (form based) I can't login anymore.
Without Request Redirection it's also working as expected.
Tested with Outlook Web Access 2010 publishing.
The WAF log shows every time I try to login:
