Mal/JSRedir-M repeatedly prompts for quarantine manager

Mal/JSRedir-M repeatedly prompts for quarantine manager. Shows up in left column under threats, but no file name/location is listed. Cleanup removes Mal/JSRedir-M from the left column but re-prompts several times per hour. I've tried uninstalling and re-installing version 9.4.1 on OS X 10.11.2, with restart between each action.

Any thoughts?
