RESOLVED: Apple has released a new version of Mac OS X 10.11.4 Beta that resolves this issue. In addition, there will be an update released soon that resolves the issue as well.

Our Engineers have confirmed that the Mac OS X 10.11.4 Beta is causing an issue with Sophos Home's On-Access Scanning and Automatic Virus Protection (in addition to other Sophos Mac products).
It has been brought to the attention of the engineers and they are currently working on this issue.

...it's possible Apple has silently introduced new requirements, like they did to everyone in 10.9.5.
While there is a temporary fix that involves disabling System Integrity Protection, this does have the disadvantages that come with disabling SIP.
