Thanks for reaching out, and welcome to the Sophos Community! 

Check out the following document: Add an SMTP route and scan policy. 

You could add a custom file type and use it within the SMTP route & scan policy under file protection. 

Note: You would have to configure email protection on the firewall to block emails with the file types. 

• Configure email protection in MTA mode
