recurring Mal/Phish-A

Greetings,

I am getting a recurring Mal/Phish-A notification from Sophos. The setting I have in Preferences is Deny Access and Move threat to users/shared/infected.

When I get the notification and open Quarantine Manager, the threat goes away as if it has been cleaned up. There is no path listed before it goes away and it never shows up in the infected folder.

Since I don't know where the threat is coming from, I'm not sure what to do prevent the recurrence, which is getting quite annoying.
